• Damaged TOC (Table of Contents) - the disc's directory structure is defective
• Defective error correction data - the Reed-Solomon codes are no longer intact
• Age-related degradation (disc rot) - the reflective layer has decomposed
• Incompatible formats - disc was burned in a format that is no longer supported
